Your Life, Your Choice! Advance Care Planning In-Person / Online
Join in a one-hour RN-led conversation on Advance Care Planning: how to make your own decisions about your future health care, and ways to talk with your family, friends, and physicians about health care choices for yourself and your loved ones. Sample Advance Directive forms will be available for you to take home. Q&A time is provided!
